download the enrollment and Moms and More Liability Release forms. The
forms should appear in your browser window. Print and fill out both forms, and follow the mailing instructions outlined in the first form. A Moms &
More membership representative will be contacting you upon receipt of the form. Annual dues are $40 for the calendar year. Dues are prorated for new
members only $20 for July 1 - December 31.
